Our Primary Care Network

Most GP practices are associated with a Primary Care Network (PCN) who provide services across a region of the UK.

Chester Central PCN

Chester Central PCN is a partnership of organisations working together. These practices come together to help people live healthier lives.

PCNs also work alongside staff, patients, their carers, families and the public to provide better healthcare. The PCN includes Chester Central practices including ourselves; Garden Lane Medical Centre along with Fountains Medical Practice, The Elms Medical Practice, Northgate Village Surgery, Northgate Medical Centre & St Werburghs.

Benefits of Primary Care Networks

PCNs have many benefits for patients, for example:

  • Patients can receive support for more complicated conditions
  • They can access multiple different types of support in their area
  • Wider types of services available

PCN ARRS Roles

To support the PCN, the additional Roles Reimbursement scheme (ARRS) was introduced. See attached flyer of what these ARRS roles are and how they may benefit you as a patient. 

ARRS roles such as Dieticians, Wellbeing Co-Ordinators, Mental Health Counsellors & Physios are all accessible via your GP practice provided through the PCN.

CVD PROJECT

You have received a text message to take part in a free 6-month wellbeing project due to start soon.

 We are working with a Health and Wellbeing Coach to develop personalised, 1-2-1 wellbeing plans for our patients.

The project supports people who are currently well but at higher risk of developing heart or metabolic conditions. It focuses on small, achievable lifestyle changes with regular support.

If you choose to take part, you’ll have a Health MOT, including checks like blood pressure, cholesterol, and a simple strength test. You’ll then receive personalised advice, and a short wellbeing call every two weeks from a Health and Wellbeing Coach
