CORONAVIRUS UPDATE:

Following Government guidelines ALL patients visiting the practice will be asked to wear a face covering and to practice social distancing.

If you present at the practice without a face covering we will have to send you away .

Face coverings are not only to protect yourself but also the safety of other patients as well as our Doctors, Nurses & other staff members.

If you have serious symptoms that you cannot manage at home, please use NHS 111 online (Please only call NHS 111 if you cannot get online).

Cough may persist for several weeks in some people, despite the coronavirus infection having cleared. A persistent cough alone does not mean that you must continue to stay at home for more than 7 days

Connecting You

"Connecting You" is an information pack for patients with palliative care needs and their carer's and families. The pack has been developed following feedback from patients and carers about the difficulties in obtaining relevant information pertinent to palliative care needs in a timely manner.

The pack contains information on a variety of topics, including services who may be involved in a person's care, details of local support groups, planning for the future, support for carers and bereavement. The pack is designed as a tool to assist patients and carers in accessing information and isn't intended to replace conversations with health and social care professionals.
